Pre-operative check list:

• Operative site marking ("OK" and/or "Provider Initials") must be performed with OR marking pen prior to patient going into the OR

• 24-hour update or daily rounding note must be present in Epic 

• Verification of consent form completed in Epic

"STOP the line"

• If there is a quality/safety concern at any time in the OR, any member of the team can verbalize "STOP THE LINE" and all members must stop their activity to discuss the concern.
